چکیده

L eft main (LM) coronary artery stenoses have conventionally been precluded from percutaneous coronary angioplasty because of the prohibitive risk of irreversible hemodynamic collapse after acute closure of the artery, and a relatively high risk of late sudden death. l When protected by left anterior descending and circumflex coronary artery bypass grafts, angioplasty of the LM coronary artery can be safely performed but is limited by a rate of recurrent stenosis in excess of 50%.* Coronary atherectomy has recently been advocated as an alternative procedure because of preliminary data suggesting a lower incidence of acute

Multivessel rotational atherectomy in left main coronary artery lesion followed by stent implantation

A 82-year-old patient with multiple comorbidities and angina, in spite of optimal drug therapy, was submitted to coronary angiography, which showed three-vessel disease with left main coronary artery involvement and severe coronary artery calcification. Coronary artery bypass grafting surgery was contraindicated and the patient was referred for percutaneous coronary intervention, which was carr...

Percutaneous transluminal coronary angioplasty with cardiopulmonary bypass for stenosis of the most proximal part of the left anterior descending coronary artery.

An 85 year old man with unstable angina pectoris was treated successfully with percutaneous transluminal coronary angioplasty supported by cardiopulmonary bypass and intra-aortic balloon pumping. Coronary angiography had shown stenoses in both the left main stem and left anterior descending coronary arteries. Drug treatment had been ineffective and he was too old for coronary arterial bypass gr...

Emergency surgical treatment for acute occlusion of the left main coronary artery-report of a case-.

We report a case of acute occlusion of the left main coronary artery (LMCA) successfully treated with percutaneous transluminal coronary angioplasty (PTCA) under the use of percutaneous cardiopulmonary bypass support (PCPS) and subsequent coronary artery bypass grafting (CABG). CABG was started only two hours after admission, and subsequent reperfusion of left anterior descending artery (LAD) a...

Results and long-term predictors of adverse clinical events after elective percutaneous interventions on unprotected left main coronary artery.

BACKGROUND The safety and efficacy of percutaneous coronary intervention of de novo lesions in unprotected left main coronary arteries remains an unresolved issue. METHODS AND RESULTS We analyzed 67 consecutive patients treated with the following devices: 39 with stents, 12 with rotational atherectomy plus stents, 13 with directional coronary atherectomy plus stents (a total of 64 patients we...

Emergency management for critical left main coronary artery stenosis.

BACKGROUND Increased experience and improvements in technology seem to have encouraged the use of percutaneous interventions for left main coronary artery (LMCA) occlusions. There is no consensus, however, and the data are inadequate on whether surgery or percutaneous procedures should be the intervention of choice for critical occlusions. METHODS From January 2002 to December 2006, 108 patie...
